Blood, Sweat & Tears: Becoming a Better Surgeon, by Philip F. Stahel

All surgeons want to be better surgeons...They work hard to be respected by their peers, appreciated by their patients, and valued by their communities. Most of the estimated 200 million surgeries performed worldwide every year go as anticipated, with positive patient outcomes. However, the number of surgical complications and preventable medical errors still remains unacceptably high. Why are experienced surgeons still creating so many adverse events? More importantly, what can surgeons do to better address the situation? This book seeks to answer these questions. The book provides pragmatic examples on how good surgeons can grow from being technically brilliant to becoming empathetic and capable of providing safe, compassionate, and more effective patient care. The book follows trauma surgeon Philip Stahel's 20-year journey from his 'rookie years' in internship and residency, to his development as a global patient safety advocate, renowned academician and teacher, and compassionate surgeon. The book touches on why our current patient safety protocols and checklists fail to keep patients safe and how a physician-driven initiative with credible leadership is needed to build a sustainable 'culture of patient safety'. Written for a wide audience and based on the paradigm that "good judgment comes from experience which comes from poor judgment", the book provides in-depth coverage of all the critical and timely components of safe surgical care, relates practical tips for improving the quality of partnerships between surgeons and patients, and offers a practical guide on how to reduce the learning curve to becoming a better surgeon.

Review
Blood, Sweat & Tears: How to Become a Better Surgeon is a remarkable book that emphasizes empathy and communication, provocatively authored by a surgeon. However, as the reader will soon discover, Philip Stahel is not your ordinary surgeon. I strongly recommend every health care provider read this book. I further recommend this book be mandatory reading annually for every medical student, intern, resident and fellow-in-training, most especially chapters 3 and 4, which epitomize William Osler's advice, "Listen to the patient - he is telling you the diagnosis". In these 20 chapters, the many other insightful quotes alone are worth the purchase price.
Jerome M.Buckley, MD
Retired CEO/Chairman, COPIC CompaniesAssociate Clinical Professor, University of Colorado School of Medicine

The life of a surgeon is difficult. Life and limb threatening problems do not necessarily occur at convenient times. Surgery is not for the weak as it requires physical strength, emotional stamina, and unquenchable intellectual curiosity. Underneath these prerequisites lies the most important of all surgical requirements: the patient. With his emphasis on patient care found through empathy, shared decision making, and attention to detail, Dr. Stahel is telling the surgeon of today and tomorrow about the way to quality improvement and self-fulfillment.

The emphasis on empathy is a crucial but neglected part of quality improvement. Why do our patients so frequently not adhere to our instructions? Putting yourself in the patient's position creates an essential surgeon-patient bond that underlies an optimal outcome. Dr. Stahel did not write the golden rule of "love thy neighbor as thyself", but it is clear that he sees this as an essential part of the surgeon-patient partnership. Both surgeon and patient will feel this effect, and it will pay dividends for both parties in the near and distant future.

It is an important but disturbing reflection that many medical students lose their empathetic qualities during their clerkship years. There are many reasons that underlie this loss including our role models, the frantic pace of clinical activities, and the lack of clear direction as to the medical student role. Importantly, Dr. Stahel gives us a path to finding our empathy by rediscovering our humanism. Relating to the janitor, the nurse, and other members of the care team as people is an important first step in understanding the common ground that we share with our patients. Letting each member of the surgical team call the professor by his first name clearly tells the staff that all are important and essential. Giving his phone number to his patients shows the trust that Dr. Stahel shares with those who trust him.

As I reflect upon my own 35-year career in surgery, I remember the eagerness with which I first approached operating room days. "A chance to cut is a chance to cure" and "the only way to heal is with cold steel" were chants that my fellow residents and I would often repeat. The operating room was its own sanctuary away from many realities of patient care. With time, I have learned to appreciate other parts of patient care. In the clinic, I have a chance to know the patient as a person, and I have an opportunity to educate the patient as I would want to be educated.

My path to becoming a better surgeon is far from over but my time to accomplish this is short. I truly wish that I had read such a book many decades ago as I began my life in surgery, but back then no such work was available. With Blood, Sweat, & Tears, Dr. Stahel has directed me to some needed tools that might help me reach this laudatory goal of ongoing quality improvement. I am most appreciative for his reflections and observations, and I remain hopeful that perhaps someday I might become a better surgeon.

Ted Clarke, MD
Orthopaedic Surgeon and CEO and Chairman of COPIC, Denver, Colorado

As a veteran Registered NurseI feel thatthis book is a must read for anyone in health care!
Dr. Philip Stahel has a very down to Earth writing style and compassionate approach to patient care. Reading this book has reinvigorated my love of nursing and passion for patient care.

Kerry Olson, RN

Blood, Sweat & Tears is a unique book - clearly one of a kind, and surprisingly not just of interest to those who work in healthcare. The book has a captivating narrative flow and the medical aspects are very easy to understand for non-clinical/laypersons as well. I will be sending my "baby boomer" parents a copy as it becoming increasingly important for the community to understand the complexity and challenges of our current healthcare system. My take-home point from this book is that we can and we should be involved in our healthcare choices and ask important and pertinent questions. If you're like me, and you're interested in patient safety and eventually receiving high quality medical care if you ever become a patient, if you have a sense of humor, and you would like a different perspective on healthcare, this is the book for you!

Nicole Morgan, MHA

From the Back Cover

I applaud Dr. Stahel for presenting a rich compilation of his honest and remarkable first-hand experiences and the collective work of doctors and health care leaders to reduce the endemic variation in medical quality that contributes to the #3 cause of death in the U.S. today--medical care itself.

Marty Makary, MD, MPH
Author of the New York Times bestseller,Unaccountable.

Blood Sweat & Tearsis a great book, one of a kind and destined to be a medical classic. What makes the book exceptional is that the narrative is about a difficult human endeavor, often done imperfectly, by humans who have been told they should be 'perfect'. This quintessential paradox is why this book is a practical story about life and will likely be of interest and enjoyment to many outside the realm of medicine.

Wade Smith, MD
Co-founding Editor,Patient Safety in Surgery.

A Great Read For Any Professional Who Works With People!
By SC
This book is outstanding, and not just for surgeons! As a practicing attorney, Dr. Stahel's insights into creating a meaningful life and practice in a high pressure professional environment are invaluable. This book contains a great mix of inspirational ideas and practical, everyday tips for thriving in high pressure environments by keeping the focus on the people we serve, and the people that we can support, even if their jobs involve supporting us. I particularly enjoyed the sections on empathy and avoiding burnout.

This is a fast read, and I expect that I will go back to reading sections of this book again and again as I progress in my own professional journey. I was challenged and inspired by this tremendous book and cannot recommend it enough!

Good information, but very much LACKING
By F.B.
I decided to write a review for this since there have been no review yet. I was taking Anatomy and Physiology as a class and needed a guide that could help explain things easier. I have heard about the Incredibly Easy series from a friend and after looking through the reviews of the Incredibly Easy series, I knew it was a good book.

I decided to go for the Incredibly Visual book for Anatomy because I thought the pictures would be helpful (my textbook already has a bunch of words in it; no need to buy another book with a bunch of words). When I got the book, it was just okay. While the pictures were in full color and very detailed, there was probably only 5 pages for each anatomy part. And I know that there isn't any anatomical books out there that will go over every body part in detail, but this book seemed to lack information.

For the price, it isn't as informational as I would like, but it is a good guide to go with "Anatomy and Physiology for Dummies" or some other guide. As a study helper alone however, you're better off reading your textbook.

Visual learners rejoice!
By emtski
For anyone who has struggled through their Anatomy & Physiology course, this is a terrific asset. It does not, I repeat, NOT, replace the hard core work of an A&P class. But for those folks who need visuals to support the words, this is an excellent choice. It also comes in handy if you are a health care professional who needs to explain physical functions.

Pathology of Multiple Pregnancy, by Virginia J. Baldwin

Pathology of Multiple Pregnancy collates case material from over 500 autopsies (including embryos) and 1200 placentas from twins, triplets, and higher multiples with selected literature on human multiple reproduction. It emphasizes important new concepts of pathophysiology and interpretation of less than ideal outcomes of multiple conception based on over 30 years experience in the field. Aspects of perinatal pathology unique to twins and higher multiples are emphasized, with practical suggestions for the service pathologist, including items of potential medico-legal significance. Controversial topics, such as the value of studies of concordance for diseases or malformations, and the significance of inter-twin anastomoses, are addressed and suggestions made for further investigation. A close working relationship between pathologists and clinical colleagues is encouraged by discussion of clinico-pathological correlations. And, the most recent discussions on mechanisms of twinning and genetic versus environmental influences on the development of twins are presented.

An important book seven years later
By A Customer
The real test of a specialty monograph is how fares over time. It is seven years since this seminal work was published. I am compelled to say, as a continuously active professional in the field, Baldwin's "Pathology of multiple pregnancy" is more valuable in year 2000 than it was when it first came out. The reason is simple: the author fairly anticipated both the basic science and the clinical spects of the related fields of infertility treatment and the neonatal consequences of polyovulation. Her compendium will deserve, but may not need, a revised edition a few years hence. The passage of time has validated her approach as a natural historian of twin studies.

China's relationship with the developing world is a fundamental part of its larger foreign policy strategy. Sweeping changes both within and outside of China and the transformation of geopolitics since the end of the cold war have prompted Beijing to reevaluate its strategies and objectives in regard to emerging nations.Featuring contributions by recognized experts, this is the first full-length treatment of China's relationship with the developing world in nearly two decades. Section one provides a general overview and framework of analysis for this important aspect of Chinese policy. The chapters in the second part of the book systematically examine China's relationships with Africa, the Middle East, Central Asia, Latin America, South Asia, and Southeast Asia. The book concludes with a look into the future of Chinese foreign policy.

China and the Developing World book review
By Gregorio Savio
The "sleeping dragon" has wakened up and is becoming a major political and economic force in regional and world affairs. Chinese diplomacy is signing investment agreements, building roads, forming strategic partnerships and gaining membership in regional and international organizations. Developing Countries are the favorite target for Beijing policy, seen both as extremely important sources of energy and raw materials, and as supporters to its multilateral approach to international affairs. China's foreign policy towards the "Third World" shifted, beginning from the early `80s, from being political-ideological to pragmatic-economic aimed. Developing countries represent for China many opportunities: sources of oil and raw materials, new markets for its products and possible allies in the United Nations and in the Taiwan's issue. This book provide a broad but still precise view of the strategies and accomplishments of China's diplomacy toward the Developing World, also mentioning the challenges that Beijing will have to face in this century and the implications for U.S.
SUMMARY OF THE BOOK
The book is divided in three sections and eight chapters. The first section and chapter is an historical introduction to China's interaction with the Developing World, from the Xia Dynasty (2206-1766 BCE) to the present. Adhering to the Confucian value system, during the Imperial era Chinese leaders focused their energies and strategies internally to secure their rule also because China was generally self-sufficient in terms of natural resources. Its self-image was one of a moral, cultural and technological superior country posed at the center of the world, as its name suggests: "the Middle Kingdom". All the foreigners were considered barbarians. The first interactions with the exterior world were obviously with the surrounding strategic periphery, mostly in the west to safeguard the Silk Road. When China was dealing with internal troubles, usually during a period of dynastic consolidation or dissolution, Chinese leaders took an essentially defensive position to hold off border threats and challenges. When stronger, they attacked the nomad barbarians and expanded the strategic periphery. In this way Chinese domain tended to expand and contract throughout history, making historic nation a geographically difficult concept to define, also because Chinese identity is based on culture rather than ethnicity. What defined a periphery state autonomous or tributary related was the military strength of the barbarians.
During the Ming period (1368-1644 CE) China began to isolate itself from the wider world outside its immediate periphery, complacent of its superiority and conditioned by antimilitaristic Confucian principles. The Empire weakened both internally and externally, particularly in terms of military power, letting the European powers and Japan conquer parts of it and compelling China to sign a series of "unequal treaties" starting from the Treaty of Nanjing after its defeat in the Opium Wars (1839-1842). This period, until the end of World War II, is considered by Chinese the "century of humiliation" that fundamentally altered China's perception of any kind of self-superiority.
With Mao's victory in the civil war and the founding of People's Republic of China (PRC), China opened a new phase in its external relations based on equalitarian ideology, focusing not only with U.S. and U.S.S.R. but also with underdeveloped and developing countries, supporting their struggles for independency. At the Bandung Conference in 1955, a meeting of 29 Asian and African states, most of which were newly independent, Premier Zhou Enlai enshrined the "Five Principles of Peaceful Coexistence". Mutual respect for territorial integrity and sovereignty, non-aggression, non-interference in internal affairs, equality and mutual benefit, and peaceful coexistence led to the creation of the "Non Aligned Movement" in which Third World nations refused to take one side in the bipolar international system headed by the U.S. and U.S.S.R.
In the 1960's China sponsored the Third World radicalism, sending arms, money, military instructors and economic aid to communist and independency insurgents in many African and South-Asian countries. Although gradually reentering the community of nations, replacing the Republic of China (ROC, in Taiwan) in the U.N. in 1971 and opening a new era in relations with the U.S. in 1972, China continued to promote anti-imperialist solidarity and economic aid based on ideology. In the late seventies, realized the economic catastrophe of the Cultural Revolution, Deng Xiaoping reformed Chinese policies to a more market-oriented economy that is still within a rigid political framework under Communist Party control. The reforms replaced collectivization of Chinese agriculture with privatization of farmlands, increased the responsibility of local authorities and industry managers, allowed a wide variety of small-scale enterprises to flourish, and promoted foreign investment. Foreign policy was now applied according to practical considerations of economic development and technology acquisition, without regard to a nation's political system or ideology. Nonetheless China continued providing military aid to Third World nations to gain capital for internal investments, buy influence, and demonstrate its continued fealty and leadership in the developing world. Pragmatism is the today priority because the Middle Kingdom is not anymore self-sufficient and has a survivor need for raw materials and energy resources owned for the most part by the developing countries.
All this long history is very important to understand the contemporary political leaders' frame of mind, reflecting a tradition of deception and a highly developed sense of political strategy to be able to gain advantage over rivals.
The second chapter examines China's post cold war strategy in Africa. Until Mao's death the main strategy toward the continent was conditioned by national security interests and the fear of a Soviet threat to China. Local fights (in Angola for example) far from the Middle Kingdom were encouraged as a diversion to keep Moscow's forces dispersed. Africa is now a component in China's larger strategy to cultivate political support, acquire energy and natural resources, secure its commercial interests and support its claims to Taiwan. Political support to China's policies is granted by $5 billion loans over three years and $5 billion China-Africa development fund that Beijing distributed in November 2006. The United Nations is the very important forum where China enjoys the support of African nations for its "one-China" policy and they can count over its veto power on censures concerning human rights violations (Sudan i.e.). China's strategy to securing raw materials and oil supplies is to gain control of African natural resources at its source to not be menaced by market prices shocks. Beijing methods to achieve African elites' benevolence are pragmatic as organizing cultural meetings, medical cooperation, tourism, bringing African students to China, agricultural and technical assistance, investment and economic support, military patronage and peacekeeping, and diplomatic support. U.S. and China have competing interests for energy and other natural resources and divergent views on democratization and human rights, but none of them wants Africa to become a region of aggressive competition.
The third chapter is dedicated to China's emergence in Central Asia. China has strategic and diplomatic interests here that play into China's overall foreign policy, such as providing a potential alternative to the Russian dominated Collective Security Treaty Organization (a collective defense institution for many Central Asian states); establishing stronger ties with its neighbors to counter in the long term a potentially hostile United States presence on its western borders. China also seeks to safeguard the stability of borders negotiated after the fall of the U.S.S.R. and has focused on preventing any external influence and support for Uyghur separatists in Xinjiang. Chinese economic and trade interests in the region, especially the development of energy resources, are also becoming increasingly important to its internal economic development. However, the U.S. and Russia still provide the largest amount of development assistance, financial aid and security assistance in the region and the gap that China is trying to fill is still pretty wide.
Chapter four present the Chinese interests in South America, focusing over relations with Brazil, Mexico, Argentina, Cuba, Venezuela and control of the Panama Canal. China and Latin America have complementary economic structures. South American natural resources are vital to China's booming economy and Chinese purchases and investments will very much help economic development in the area. The Middle Kingdom looks to Latin America for supply of oil, iron ore, copper and other raw materials, along with foodstuffs such as soybeans, grain and beef meat. Chinese leaders have emphasized summit diplomacy and "win-win" relations to cultivate local support for expanding economic relations and market penetration of Chinese cheap goods. China's strategic moves and economic diplomacy will inevitably present challenges to the United States, also given that anti-Washington sentiment have increased in the region.
Chapter five explains China's interests and strategy in the Middle East and Arab countries. The main interest is obviously the stability of the region to not put in danger the extensive oil supplies. Furthermore, the M.E. and Arab world are important markets for Chinese products and services, and the Chinese government counts on the region to continue supporting the one-China policy related to Taiwan. To promote its interests, China has adopted strategies such as the use of diplomacy to help Chinese businesses make inroads to the oil industry and markets, the adaptation of an offend-no-one approach to regional politics in order to maintain a friendly environment to Chinese business interests, and the use of soft-power resources to cultivate friendship also in the long term. But China does not have the resources to maintain stability in the area to secure its strategically economic interests, and it has to rely on the United States as the primary force for peace. That is why a priority is not to confront with the U.S. over regional issues.
Chapter six and seven deal with China's strategies and relations with South and South-East Asia. In South Asia the main actor that China faced is India. In the past they had prolonged territorial disputes that also drove to a war in 1962 together with the Tibet issue. Since the sign of the "Agreement to Maintain Peace and Tranquility along the Line of Actual Control" in 1993, political relations have been distensile and the remaining border disputes are not worth of war. Despite official statements that India has accepted China's sovereignty over the Tibet, the Tibetan issue remains contentious because of the spiritual and political influence of the Dalai Lama residing in Dharamsala, India. Anyway China is warming up relations with the subcontinent, fearing an U.S.-India alliance to contain it. China and India have growing economic and trade ties that contribute to an improving security relationship, but their status of cheap producers is also making them economic competitors. Southeast Asia is considered the back yard by Chinese leaders, and the Middle Empire has always had a big influence and power of intervention over these countries' affairs. In the present China is focusing on its internal economic development, trying to secure access to raw materials in Southeast Asia such as oil, natural gas, rubber, and tin. For these reasons ASEAN nations are worried about a return to a colonial economic relationship with Beijing. China's future military modernization, especially if it is combined with more threats against Taiwan, are likely to make it more difficult to believe that Beijing has only peaceful intensions in the region. At the moment the U.S. and Japan remain better positioned to wield considerably greater influence in Southeast Asia, thanks to higher trade and investments levels. Anyway China's increasing influence in the area is a challenge that United States and their allied will need to confront in the next decade.
The eight and last chapter is devoted to the evaluation of China's strategy toward the whole developing world, pointing out that Beijing new diplomacy is more proactive and flexible. It has embraced multilateralism and confident building measures to mitigate security concerns, it has identified several states and regional organizations as strategic partners, it has provided economic assistance strategically to further bilateral relationships and it has emphasized and cultivated soft power.
The main purpose of the book is to respond to four sets of questions. Each author aims to identify how China defines and pursues its interests in the region under question, which methods and assets are brought to bear to achieve policy objectives, identify relative successes in meeting policy goals, and examine the implications of Chinese foreign policy for other regional and global actors (mostly the U.S.). The book is very effective and explicative in exposing the different issues China is managing, and definitely maintains what it promises in the introduction. I found very interesting the shift from an ideology based foreign policy to an economic based and the major concern Chinese diplomacy has to secure raw materials supplies. It seems that every diplomatic effort is made in synergy to ensure this goal. I also find the book complete and exhaustive in presenting China's strategies for the developing countries in the twenty-first century even if everything is presented in almost 250 pages.

RECEPTION, CRITIQUE AND RELEVANCE OF THE BOOK
Since this book has been edited in August 2007 I did not find any exhaustive review. Anyway the few comments that I found concord over considering it the first comprehensive treatment of contemporary Chinese diplomacy's strategy in the developing world. The commentators underline the relevance of China's growing use of soft power to win friends in key emerging regions and the comprehensiveness of data presented, together with the sharp analysis provided. I agree with these comments and I think that the book is covering the lack of knowledge about Chinese strategies present in the Asian field of studies. It is well written, easily understandable and full of profound insights. The division in chapters associated to a particular region, and the chapters' internal schematization of the intensions, actions and implication of China's policies make it a perfect manual for International Relations students and readers.
The authors are not providing predictions of future China's effect on the international system, but they depict probable scenarios based on the analysis of present policies and strategies. It is important to notice that they do not consider China's emergence in the recent years coming merely at U.S. interests' expenses. They see the Middle Kingdom both as a challenge and an opportunity for the international community. They do not have a closed view of power as a zero-sum game, but they consider also the possibility of win-win relations.
If I may find a weakness in the book, I think that the authors should have dedicate a more detailed section to expose the psychology behind Chinese leaders and their millenary tradition to deception's practices in carrying out foreign policies. As Mei Yaochen, a commentator of Sun Tzu's Art of War stated "Without deception you cannot carry out strategy, without strategy you cannot control the opponent". The strong insights that the book displays are many: the already mentioned structural organization of the book, the view of China's foreign policy as a quest for strategic and economic influence, the centrality in Chinese interests of acquiring raw materials and energy resources, the preeminence in leaders' worries of borders' security and internal and external stability, the use of soft power and regional cultural and economic organization to build confidence about China's aims and actions, the recognition that China's trade and investments are in the developing countries are devoted to allow Beijing meet its domestic needs, the Chinese strategically avoid to directly confront the U.S. and the seek to expand Chinese influence in the Middle East and the Arab world, the acknowledgment of the progress made by India and China toward building a strategic partnership and the recognition of an existing structured Chinese foreign relations agenda for the developing country that U.S. does not seem to have at the moment. This volume places the recent developments of China's diplomacy in the broader context of its overall foreign policy and history of relationships with developing countries.
The book is also very relevant to the field of United States foreign policy. Every author addressed the following questions: "What impact does Beijing recent engagement have on U.S. interests? And what impact does it have on the preservation or evolution of international norms and institutions?"
As the major world's actor, the United States are always considered and mentioned along the chapters and Chinese policies always take in consideration Washington policies and interests in the different regions. Even tough the top front target of China's diplomatic agenda is to secure resources to develop economic evolving national interests; China does not want to confront the U.S., at least in the present period. The book also presents suggestions for American foreign policy, responding to Chinese protagonism. The authors analyze American and Chinese interests pointing out where they collide or they coincide, very often advising U.S. to pay closer attention to the development of China's relations with the developing world. The Middle Kingdom has many chances to become the second major world power in the twenty-first century, and U.S. diplomats and politicians need to keep constantly an eye on its moves.

Realize a limitless vision and create the life you've always wanted!

Experience true bliss and abundant happiness!

Each of us has two "aspects" within us. One is the ego; the other is the higher or spiritual self. The ego, which protects and enhances our physical self, is too often the dominant power. It drives our career ambitions and many of our goals while confining us to a life of striving. Any thoughts or beliefs that interfere with our functioning on a spiritually satisfying level can be traced to the ego and its relentless demands.

Freedom is the ability to exit the single room of awareness you were born into and enter the limitless space of heightened awareness. Developing this awareness is the only way to awaken the dormant spirit - the higher self - and experience true bliss and abundant happiness.

In Freedom Through Higher Awareness, Dr. Wayne Dyer provides you with a practical path to the higher self, showing you how to leave the constraints of daily awareness and realize a limitless vision.

Wayne teaches you how to discard old beliefs and opinions that clutter your mind and make space for a vibrant new energy - one that's genuinely enlightened, fully functioning, and anxiety-free!

Freedom Through Higher Awareness will also help you:

  • Manifest your dreams in reality and discover your driving desire.
  • Experience bliss in your everyday life and contribute to the bliss of the world.

When you experience higher awareness and discover your true identity, you'll not only be able to "see" the life you've been longing for, you'll be able to create it!

Life Changing
By smithslove
My therapist recommended this. She said this was one of the first spirituality tapes that she had ever purchased and it changed everything for her. It is unfortunate that this is not on CD so that I can take in my car. Also, it could really help a lot of people even these days. But, I listen to it a few minutes a day with my old boombox when I am making breakfast and packing my lunch.
I love his practical examples to change your life on a daily basis. I practice these daily. There are some parts on the tapes at the end which is difficult for me to understand, but getting the main jist is definitely enough. I am part of the hayhouse website which has other spiritual gurus, but I cannot get into them. Dr. Wayne really is someone that speaks to the heart. This is way worth the $3 you can get it for.

Since I found a distribution of skill and helpfulness in the essays and discussions, the easiest way to approach this is to review each of the authors separately.

Dr Hoyt is a dispensational seperationalist whose eschatology leads him to a position that Christians should be personally against violence but should not try to discourage their country's war effort and can even join it in a non-combatant role. The weakest position also happened to get the weakest defender. Dr Hoyt is a sincere old time Bible preacher with obvious passion for the things of God and the mission of the church...but he is not a skilled debater or a convincing author and is generally outmatched throughout the discussion. Augsburger briefly summarized the position in order to distinguish his from it and gave it a better treatment in 2 paragraphs.

Dr Augsburger is a much more formidable presence (perhaps the most formidable presence of the four) as the voice of pacifism and makes a stirring case for active non-violent intervention after the style of the cross and Dr King. I could not help wanting him to be right. And regardless of if he is, he does his job in championing the strict Sermon on the Mount ethic which any Christian has to come to terms with.

Dr Holmes, in his defense of the just war position, made the best use of the format. He was gracious but pointed in his comments. He stayed on message, effectively conveying a concise and immanently defendable thesis before we even got to his chapter. His defense of the just was accompanied by an appropriate denunciation of war and all its evils and a competent ethical stance of selecting a lesser evil. There is an issue of uncertainty of outcomes (e.g. will a `lesser evil' actually produce less evil) that was not fully dealt with, but Holmes message was coherent and well presented.

Dr Brown was an able proponent for a difficult position (crusade/preventative war) but was also a bit aggressive and condescending. He skillfully deconstructed his co-authors eschatologies to undermine their positions, which simultaneously provided a helpful service and left me unsettled. It was his flirtation with ad hominim calling Dr Augsburger dangerous and delusional that disappointed me the most. At the heart of his argument seems to be the idea that if defensive action is justified, preemptive action is justified on precisely the same criteria. It was more successful than I thought it had a chance of being. However, while he dealt briefly with the problem of imperfect future knowledge (how does one know that preemptive attack is necessary) he did not overcome the difficulties it poses for the equating of preemptive militarism with the reluctant acceptance of the evils of a just war. In the end Dr Brown did all right with a fundamentally flawed position

It also should be noted that it was written in `81 and the aftermath of Vietnam weighs heavily on the discussion. On the whole though, it is a worthwhile and timely book even 25+ years later.

A good start for a Christian engaging this issue
By Robert Burns
Each of the four authors writes an essay defending his position, and then the other three respond, making for an interesting dialogue.
All four make compelling cases, partly because the New Testament does not directly address the issue of what a Christian or Christian community's response to war should be. The history of the Christian church will show how very differently popes, rulers, and Protestant communities viewed involvement in war.
Augsburger makes a compelling case for pacifism. What is attractive about pacifism is that it so brilliantly proclaims to the world a different way, the Way of Christ Himself who eschewed violence against those who mistreated him. The community then places its faith only in God for its protection. This position thus is naturally connected with the refusal to be politically involved, which it has historically accompanied in the Mennonite and other pacifist communites.
Hoyt says Christians can serve their country, but only in non-combat roles, as the Christian is bound not to perpetrate violence to another human. I must admit I was strongly attracted to this - and I'm sure this probably has reflected the views of many conscientious Christians once drafted. But this breaks down, because it sets up a double standard.
Holmes presents the classical doctrine of "just war". The problem with this is that a "just war" is hard to come by. Acting completely in accordance with it requires that no military forces attack territory or citizens belonging to the aggressor -- only retaking territory the enemy has taken -- thus being purely defensive. Reality tends to make conducting a "just war" impractical and unwise -- the Allies would have stopped at Germany's borders in World War II.
Brown defends the ideas of the "crusade" and the "preventive strike". This may sound offensive and did to me, but Brown is capable. But the problem lies in the Christian's justification of the particular crusade or strike. And, of course, how is this different from how the world conducts its warfare?
There are two fundametal realities that we confront as Christians facing war: One reality is that there are unscrupulous, evil, and aggressive rulers who need to be checked. The other is that war is an incredible evil and can do nothing but wreck the conscience -- which is especially troubling for the obedient Christian who knows that human life is made in the image of God. So how do we trust and obey God in the midst of war? The state "bears the sword"; can a Christian thus be part of such a state? If so, how should the state bear it?
I continue to struggle with the answers, but heartily recommend this to someone needing a beginning point on this.

In a city so dismal it's known only as "the Hole," a clan of Sorcerers has been plucking people off the streets to use as guinea pigs for atrocious "experiments" in the black arts. In a dark alley, Nikaido found Caiman, a man with a reptile head and a bad case of amnesia. To undo the spell, they're hunting and killing the Sorcerers in the Hole, hoping that eventually they'll kill the right one. But when En, the head Sorcerer, gets word of a lizard-man slaughtering his people, he sends a crew of "cleaners" into the Hole, igniting a war between two worlds.

Inside the pool of waste, evil begins to brew. The En Family leaves the hospital in search of a new hideout. They head for the Middle Department store, which has turned into a dark, warped skyscraper for the once-a-year Living Dead Day. When Ebisu and Kikurage are separated from the group, the rest of En Family searches for them inside the store.

Programming .NET Security: Writing Secure Applications Using C# or Visual Basic .NET, by Adam Freeman, Allen Jones

With the spread of web-enabled desktop clients and web-server based applications, developers can no longer afford to treat security as an afterthought. It's one topic, in fact, that .NET forces you to address, since Microsoft has placed security-related features at the core of the .NET Framework. Yet, because a developer's carelessness or lack of experience can still allow a program to be used in an unintended way, Programming .NET Security shows you how the various tools will help you write secure applications. The book works as both a comprehensive tutorial and reference to security issues for .NET application development, and contains numerous practical examples in both the C# and VB.NET languages. With Programming .NET Security, you will learn to apply sound security principles to your application designs, and to understand the concepts of identity, authentication and authorization and how they apply to .NET security. This guide also teaches you to:

  • use the .NET run-time security features and .NET security namespaces and types to implement best-practices in your applications, including evidence, permissions, code identity and security policy, and role based and Code Access Security (CAS) use the .NET cryptographic APIs , from hashing and common encryption algorithms to digital signatures and cryptographic keys, to protect your data.
  • use COM+ component services in a secure manner
If you program with ASP.NET will also learn how to apply security to your applications. And the book also shows you how to use the Windows Event Log Service to audit Windows security violations that may be a threat to your solution.Authors Adam Freeman and Allen Jones, early .NET adopters and long-time proponents of an "end-to-end" security model, based this book on their years of experience in applying security policies and developing products for NASDAQ, Sun Microsystems, Netscape, Microsoft, and others. With the .NET platform placing security at center stage, the better informed you are, the more secure your project will be.

Out-of-date but still useful
By Timmy_A
Programming .NET Security covers 2 major topics of .NET Security: Code Access Security (CAS) and Cryptography. (CAS) was highly advertised feature of .NET when first editions of framework were released. Main goal of CAS was to provide highly configurable way how to protect your applications against malicious code. CAS as it was designed became quite complex and the book did a great job to explain how it works and how can be configured. Part 2 is mostly devoted into this technology. Unfortunately complexity of CAS forced Microsoft to change it substantially. Form .NET version 4.0 many parts of CAS are deprecated now. Especially Security Policy which was completely removed from the framework. So chapters 8 and 9 dealing about Security Policy are obsolete now. Chapter 6 Evidence and Code Identity is partly valid (Evidences are still part of .NET but because of Security Policy removal their importance is lower). Chapter 7 dealing with Permissions is mostly valid - just few methods of Permission class became obsolete. Chapters 10 and 11 (Role-Based Security, Isolated Storage) are completely valid in .NET 4.5 and provide good intro into these technologies.

Part 3 is all about Cryptography. This is the most useful part of the book covering Hashing, Symmetric and Asymmetric algorithms, Key exchange and Digital signatures. All cryptography classes presented in the book are part of latest edition of framework just few more algorithms have been added. Cryptography chapters are not useful just for their API coverage, but also because they provide excellent introduction to cryptography itself. Algorithms are presented in understandable way with nice, clean figures.

Part 4 is about security aspects of ASP.NET, COM+ and Log service. Honestly just last chapter (Log service) is useful. Coverage of ASP.NET and COM+ is just on very basic level and COM+ is now in fact dead technology. Last part is reference to all security-related classes. These chapters are not much useful. MSDN documentation is sufficient.

The book uses similar pattern in most chapters. First few pages in each one give you theoretical introduction into technology. After that, there is a presentation of corresponding .NET API and most chapters are finished with complete solution how to customize presented technology. For instance the book contains implementation of custom symmetric and asymmetric algorithms. This pattern is little bit 'dry' especially in Part 2 and 4 but very helpful in cryptography chapters where solid theoretical background is must before you can move to API.

So, I can say, that even 10 years after publication some parts of Programming .NET Security are still valuable. Especially part 3 Cryptography is perfectly valid in .NET 4.5. If you need to use cryptography in your projects, the book gives you great intro into it.

ALERT - This is a must have book!!
By Rex
You really are not a true .NET Programmer until you understand the security mechanisms that are part and parcel with the framework. To program in .NET (or really any component-oriented technology) without security in mind is like parachuting without one strapped to your back.
I was waiting for a book like this. Before this book I've had to scour over the internet to try to find out how to get the different areas of security in .NET to work. Now it's all here in one book. The theory, the explanations, the warnings, the samples. If you are a serious .NET programmer or .NET policy administrator then this is a must have book. If you don't know the difference between "host evidence" and "assembly evidence", then you need this book. If you don't know the difference between a "security demand" and a "permission request", then you are dangerous to the people you do work for.
Also, make sure you know the basics already of the language and the framework since this book assumes you do. Good luck.

Best .NET security book I've seen
By ueberhund
While there is a lot of talk about .NET security, relatively little can be found in terms of documentation, which is one reason why this book is so refreshing. In addition to discussing some of the reasons behind certain security schemes, the theory is explained as well as the C# implementation. While there are some .NET specific security issues discussed (e.g. configuring worker processes), the section on cryptography should be required reading for everyone in the computer industry.
I get really excited about a book when it contains a lot of good information and I am able to actually use it to solve real-world problems. After reading this book, I was able to help solve a really tricky (and politically challenging) security issue quite quickly. If you have anything to do with your company's security systems or write any .NET code, I think this book deserves a place in your reference section. This is certainly the best book on .NET security I have read thus far.
